Privacy Policy

What we collect through this website, why, and the choices you have.

Draft for legal review. This policy is not yet in effect.

CPT Network Solutions, Inc. (“CPT”, “we”, “us”) runs this website. This policy explains what information we collect through it, how we use it and the choices you have. It covers this website only. The CPT Care customer portal at cptcare.com has its own terms.

Information you give us

We only collect information you choose to send us through a form on this site:

  • Consultation requests. Your name, company, email address, phone number (optional), the service you’re interested in, your number of locations and your message.
  • Newsletter sign-up. Your email address. We send a confirmation email first, and you’re only subscribed once you confirm.
  • Field Service Partner applications. Your company details, addresses and locations, contact and accounting contacts, the services and skills you offer, licenses and certifications, rates and payment terms, and the documents you upload: certificates of insurance, your W-9 (or W-8) tax form and any license or certification files. A W-9 contains your taxpayer identification number. To start or resume an application, you sign in with a link we email to you.

Information collected automatically

When you submit a form we record the page you submitted it from, any campaign (UTM) parameters in the link you followed, and the time. To protect our forms from abuse we use your IP address to limit how often a form can be submitted, and we may use Cloudflare Turnstile to check that a submission comes from a person.

This site uses a session cookie that is needed for forms to work securely and, for partner applicants, to keep you signed in. We don’t use advertising cookies.

How we use it

  • To answer your consultation request and follow up about our services.
  • To send the newsletter you subscribed to. Every newsletter has an unsubscribe link.
  • To review your partner application, contact you about it, set you up as a Field Service Partner if approved, and pay you for work you do for us.
  • To keep the site and its forms secure and working.

We don’t sell your personal information.

Who we share it with

We use service providers to run the site, and they handle your information only on our behalf:

  • Email delivery: Postmark sends our form confirmations and notification emails.
  • File storage: documents uploaded with partner applications are stored, encrypted, in Amazon Web Services (Amazon S3).
  • Hosting: the site and its database run on servers hosted by DigitalOcean.
  • E-signature: partner agreements are signed through DocuSign.

We may also disclose information when the law requires it, or to protect our rights or the safety of others.

How we protect it

Uploaded documents are stored encrypted and are only available to the CPT staff who review partner applications. W-9 forms and taxpayer identification numbers are restricted to our partner team and are never included in emails. Staff access requires signing in with a CPT account.

How long we keep it

Unfinished partner applications are deleted 14 days after your last activity. We keep other information for as long as we need it for the purposes above, or as the law requires.

Your choices

You can ask us what information we hold about you, ask us to correct or delete it, or unsubscribe from the newsletter at any time. Email support@cptnetworks.com or write to us at 950 IL 83, Suite F, Wood Dale, IL 60191.

Children

This site is meant for businesses and isn’t directed to children under 13. We don’t knowingly collect their information.

Changes to this policy

If we change this policy, we’ll post the new version here with a new effective date.
